Thank you for keeping our community safe! We are glad that you are interested in receiving a COVID-19 vaccination. 

In order to schedule an appointment to receive your COVID-19 vaccination, the Merced County Public Health Department has developed a COVID-19 Eligibility Certification form which will ask a series of questions designed to determine the phase and tier that you are in. Once your phase and tier is approved by the State of California AND vaccine supplies are made available by the State of California, you will be contacted via email by the Merced County Department of Public Health and notified of the locations available to receive a COVID-19 vaccination. At that time, you will schedule your vaccination with one of the available locations. 

Submission of this form does NOT reserve a COVID-19 vaccine unless you are in Phase 1A or are 65 years of age or older and vaccines are available.

If vaccinations are available for your phase and tier when completing this form, options will be made available upon submission. The COVID-19 vaccination requires two doses, so please continue to monitor your email account as we will provide ongoing updates as additional vaccines are made available by the State of California.
